CONSENT FORMS
These Forms are available for you to print ahead of your appointment. We do tattoo minors (16 and up) with parental consent. This requires both the minor consent form and the and the regular tattoo consent form to be filled out and notarized. Both the parent or legal guardian and the person being tattooed must have a VALID government issued photo ID. The parent or guardian must be present during the entire tattoo process.
